2025 CHAD / GPHDN Annual Conference: A Celebration of Collaboration, Leadership, and Innovation

The Community HealthCare Association of the Dakotas (CHAD) and the Great Plains Health Data Network (GPHDN) are grateful to all who attended the 2025 CHAD/GPHDN Annual Conference, “Health Centers: Sustain, Grow, Thrive.”

Health center leaders from across North Dakota, South Dakota, and Wyoming came together in Fargo, ND, from June 3–5 for three days of connection, learning, and renewed purpose.

The conference opened with a full day of pre-conference sessions focused on leadership development and peer learning. Over the following two days, attendees engaged in dynamic sessions designed to address the evolving needs of community health centers—from strengthening workplace culture and leadership skills to enhancing care management programs and navigating financial sustainability.

There's Still Time to Share Your Story

Thank you to everyone who participated in the NACHC story collection project at the conference. If you didn’t attend, didn’t get a chance to record a video, or just needed more time to think about what to say, we’re happy to share that you can still record a message from the comfort of your own device. If you follow the links below, there will be instructions for how to record on your own. Your story can help us share the importance of health centers to your patients and your community. We’re hoping to get 10 more stories to reach our conference goal.

  1. Why is continued investment in health center funding important to your community? Click here.
  1. Why is Medicaid coverage important to your patients and your health center’s sustainability? Click here.
  1. Describe a program or service funded by 340B at your health center and why it is important for patients. Click here.
